You may apply online using UF Graduate School's online application.
Please be advised that you are still required to send the following materials to the UF Department of Occupational Therapy, Distance Learning Program, PO Box 100164, Gainesville, FL 32610:
- Departmental application form.

- Immunization form
and 1 photocopy.
- Varicella (Chicken Pox) confirmation form.

- Official Transcripts (2 official copies) from all undergraduate and graduate institutions attended. Applicants who do not hold a Bachelors Degree in Occupational Therapy must verify where and when they completed their OT education.
- Copy of OT certification or OT licensure.
- GRE report (copy).
- TOEFL scores (copy - international students only).
- Statement of Purpose concisely expressing interest in, and specific qualification for, being a leader in the field of occupational therapy and for independence in distance education graduate work. You should also cite one or two possible areas of study for your Professional Development Project.
- Three (3) letters of recommendation, one (1) of which must be from an OTR, addressing applicant's leadership potential, ability to work independently, and potential for success in graduate work. (Please ensure contact information, address, phone number and e-mail are provided on application forms for these references). All letters should be current, professional, written on letterhead and specific to your application to this program.
